In practice, quite a lot, because very few people exercise that possibilty. For iPhone/iPad users, unless they jailbreak their device (and I think it's a much smaller group than people who sideload apps on Android), it effectively means the app is non-existent. So, if you already have the app, you'll probably need more effort to get more of your friends / relatives into it.
